MFA Accommodation Request

Submit this form if you have accessibility needs or other difficulties using the available MFA options. ITS and CAL will review your request and provide a suitable alternative authentication method. Please note, the following MFA methods are available to all students and employees: Microsoft Authenticator app (recommended for most users), Security Key (e.g. a YubiKey that you can purchase), and One time Passcode via SMS, email, or phone call.
